SIMPLEPRACTICE ELECTRONIC HEALTH RECORD: Harbor Psychological Services, PLLC uses SimplePractice to electronic health records, paperwork (e.g., consent forms and questionnaires), billing statements, and administrative communication. SimplePractice also facilitates appointment reminders, email correspondence, text notifications, and secure messaging for administrative matters. All communication via SimplePractice is limited to administrative and billing related issues and does not include assessment, treatment, or emergency services.
You are responsible for reading and understanding SimplePractice Terms of Service, Privacy Policy, technical documentation, and any other relevant communications that SimplePractice provides to clients. You do not assume that Harbor Psychological Services, PLLC has access to any or all of the technical information for SimplePractice, or that such information is current, accurate, or up-to-date. You will not rely on Harbor Psychological Services, PLLC to provide technical information regarding SimplePractice Terms of Service, Privacy Policy, services, or any other feature or action conducted by SimplePractice. You must direct any questions regarding SimplePractice policies and procedures to SimplePractice support. Harbor Psychological Services, PLLC cannot provide technical guidance beyond the basic administrative functions we use.
TELEHEALTH SERVICES: If you choose to receive telehealth services, you will be required to sign a separate Consent for Telehealth Services before scheduling a telehealth ointment. The policies, benefits, and risks of telehealth are outlined in that consent form.
ELECTRONIC COMMUNICATION: Electronic communication methods are available but not required for you to access services. If you prefer to communicate by phone and U.S. Postal Service, you must indicate your preference in writing to Harbor Psychological Services, PLLC. Please remember that services will not be scheduled until we receive all required paperwork.
Though Harbor Psychological Services, PLLC will make every effort to maintain confidentiality in electronic communications by using passwords, encryption, and HIPAA compliant applications, confidentiality through any electronic form cannot be guaranteed. Electronic communication methods, including email, text messaging, and secure messaging, carry certain risks. These risks include but are not limited to: unauthorized circulation, forwarding, or storing of your information in electronic files, accidental release of your information to unintended parties, fraudulent documentation created about you from parties unauthorized access to your information, use of your information as evidence in court proceedings, and inspection of your information by employers’ services or other online services you have used to transmit your information.
You agree to use electronic communication only for administrative matters, such as appointment reminders, billing questions, or scheduling. For clinical concerns or sensitive questions, contact our office directly by phone. By providing your email address, you agree that Harbor Psychological Services, PLLC may contact you via email regarding nonclinical matters.
Harbor Psychological Services, PLLC reserves the right to terminate electronic communication at its sole discretion at any time. You may also terminate electronic communication at any time. If either party terminates electronic communication, further communications will be by phone or U.S. Postal Service.
Even if you choose not to use electronic communications, I may at times be required to use email or to forward email, such as billing inquiries or insurance-related questions, to a third party billing company, payment processor or other parties for billing, payment, or other administrative purposes. You agree to allow Harbor Psychological Services, PLLC to communicate by email or to forward email as required for such billing or administrative purposes.
