Electronic Signatures and Online Forms

For your convenience, we offer the option for you to complete and sign required documents electronically through SimplePractice.

Using an electronic signature is entirely optional—choosing not to sign electronically will not affect your ability to receive services. If you prefer, forms can be mailed to you instead. However, all required forms, whether signed electronically or on paper, must be completed and returned before your first appointment can be scheduled.

If you choose to use online forms with electronic signatures, you must first sign an electronic signature consent confirming that you understand and agree to the following:

Your electronic signature is legally binding.

At the time of signing, you are physically located in North Carolina.

If the client is a minor or dependent adult, you have legal authority to sign on their behalf.

Harbor Psychological Services, PLLC will keep a copy of all electronically signed documents.

You have the right to obtain and save copies of documents you sign electronically.

You may request a paper copy of any electronically signed document by submitting a written request.

You can withdraw consent for electronic signatures at any time, except for documents already signed.

Your ability to receive services won’t change if you choose to stop using electronic signatures, as long as you provide physical signatures as required.

To revoke consent, you must send your request in writing to Harbor Psychological Services, PLLC.

You will need to verify your identity by presenting a legal form of identification, both uploaded to SimplePractice and in person during your services.

You agree to promptly notify Harbor Psychological Services, PLLC of any errors in electronically signed documents.

Any request for a paper copy of electronically signed documents applies only to those documents, not others generated during your services.

You are responsible for reviewing SimplePractice’s Terms of Service, Privacy Policy, and Business Associate Agreement to understand their policies and procedures regarding your information and the use of electronic signatures.
