- YOU MUST BE ON TIME. WE WILL NOT SEE LATE ARRIVALS.
- WHO MAY ATTEND: You may bring only ONE other person into the office if you need help with forms. Otherwise, no extra guests or siblings are allowed. In addition, restrooms are for clients and their one approved guest only. If other guests interrupt, your appointment will be cancelled.
- IF THE APPOINTMENT IS FOR SOMEONE UNDER 18 YEARS OF AGE: A parent or legal guardian MUST attend with any claimant under age 18. Legal guardians must bring their court documents proving guardianship. Children in DSS custody must be accompanied by their social worker. If you do not follow this policy, your appointment will be cancelled and will not be rescheduled, no exceptions.
- IF THE APPOINTMENT IS FOR AN ADULT WHO HAS A LEGAL GUARDIAN: Legal guardians must bring their court documents proving guardianship. If you do not follow this policy, your appointment will be cancelled and will not be rescheduled, no exceptions.
- EVERYONE who enters the building may be required to wear a mask covering mouth and nose for the entire visit, depending on local health concerns. If we require a mask, we will provide you with one.
- IF YOU NEED TO RESCHEDULE: You must contact Disability Determination Services at 1-866-542-8113. I am not able to schedule appointments directly. You must provide at least 24 hours notice or you will be considered a no-show.
- No-shows may not be rescheduled. If you cannot attend, you must contact Disability Determination Services before your appointment. Failure to call is considered a no-show. If you cannot reach Disability Determination Services, you may call my office.
- Please bring glasses, hearing aids, and any other items you need to be able to complete paperwork, take paper and pencil tests, and complete an interview. If you need help with reading or writing, please bring someone with you who can help. The examiner is not able to complete your paperwork for you.
- No recording is allowed. Recording violates test security and privacy rules. Any recording will be reported to Disability Determination Services and your exam will be stopped immediately. Violation of this policy may also result in legal action against you.
